What do you need to do?

To ensure that your child is enrolled at Greenvale Primary School for 2019 you need to:

Attend one of the enrolment sessions at the school.  If possible please attend the tour/enrolment session without young children as it a lengthy process.

  • You must bring along your child’s Original Birth Certificate or Passport which will be sighted and returned to you.  A photocopy of the Birth Certificate or Passport will be retained by the school.  Your childs enrolment will not be accepted until we sight the birth certificate or passport.
  • Supply the completed School Entry Immunisation Certificate which is sent to you from the Australian Childhood Immunisation Register (Phone: 1800 653 809) by the start of school year.  A photocopy of the School Entry Immunisation Certificate will be retained by the school.
  • Complete the prescribed Enrolment Application Form, which will be available at the above sessions.  You cannot take this form home, it must be completed and remain at our school.
  • The enrolment form is 8 pages long so please allow enough time to complete the form.
  • Due to our growing enrolments preference will be given to students where Greenvale Primary School is their closest school

 

The information you will need to complete the Enrolment Application Form is:

  • mother and father’s work information – occupation, name of company, address and telephone number
  • at least two emergency contact people other than parents e.g. aunt, uncle, grandparent or family friend
  • any court orders or custody restrictions pertaining to your child
  • family doctors information, Medicare number, ambulance member
  • medical details about your child, any medical reports

To be sure that your child is eligible for school, the regulation states the child must turn 5 on or before 30th April 2019.  (Any concerns in relation to this regulation must be raised with the Department of Education and Early Childhood Development.)
